Payments for dental services are due at the time of service.
Dr. Crawford's office works with most major insurance providers.
Our business staff is glad to assist with inquiries concerning your insurance coverage. A partial list includes: Blue Cross Blue Shield Preferred and FEP; Aetna PPO; Cigna PPO; Delta Dental PPO and Premier; Guardian; Met Life, Southland, DentaMax and more. We gladly file your insurance for you and accept assignment of benefits; however, any co-pays and deductibles are due at the time of service.
We accept cash or credit cards: VISA, MC, Discover, and American Express. We also accept checks when presented by the account holder with a valid driver’s license.
Our office is proud to offer our patients convenient financing options to fit any budget. Upon approval, payment arrangements may be made through Wells Fargo Financial and/or Citibank with terms up to 12 months interest free. Extended terms are also available up to 24 months. The application process is quick and easy.
